With that said it's not recommended to play the DOS version in DOSBox because it will be very slow (depending on the speed of your computer) Also you will not be able to apply any of the patches that have come out for the game since it's release. IIRC there was only one unofficial patch release that worked for the DOS ver.

* Now Aradesh correctly reacts to player's apologies (his dialogue reaction goes up).
* Restored the original developers idea of Aradesh reaction to pickpocketing: now he doesn't attack player, only warns him and lowers his dialogue reaction.
* Removed a confusing option 'I need more supplies before I can tackle those guys.' in Seth' dialog. (May be he was intended to actually give you some stuff, but he doesn't.)
Junktown
* Tycho no longer 'forgets' character's name.
* Now Neal correctly opens Skum Pitt door.
* Now Neal reacts if player tries to mess with the Skum Pitt door.
* Option 'Actually, can I ask you some questions?' in Trish dialog removed for characters with IN lesser than 6, because all of the next options require IN more or equal than 6.

Hub
* Now cops correctly address male and female characters.
Necropolis
* Now you correctly receive 25XP for picking the lock to room with imprisoned ghoul in Watershed.
Military Base

* Abel now has correctly working Field Switch in his inventory.

And another one news. Here you may find a 3rd release candidate of unified 1.2.1 patch:
All fixes from this version are already included in 1.3.5 unofficial patch, so if you own an US release, you probably will not want to install this patch. Use 1.2 semi-official version (I recommend it for all US version owners). But if you experiencing problems with TeamX patches, I strongly recommend to download and test 1.2.1 release candidate. It seems, all tested non-US versions (see readme for the details) are much stable with this one.

It's not surprising if you miss TeamX stuff as their site is russian only these days so I too get their updates from NMA instead of their site. Calling TeamX a team is also streching it as it has pretty much been reduced to just Wasteland Ghost and possibly on other person (Alchemist I think) who he has not been in contact for some time now so it's pretty much one man effort now with help from community like Killap's projects.
Oh and he's working on Fallout 1 Restoration Project now as well and has completed first two parts of it and is working on third and last one:

LA is the special town for Master, because of the Followers. When it's invaded,
in the Followers library you'll see not only mutants, but also some Childrens
of the Cathedral, trying to gain an understanding of Followers legacy.
Game ends with the invasion of Vault 13. Don't wander too long in the Wasteland!
I lost my savegames when updating the patch from v1.3.2 to 1.3.4 with the White Label DVD version of the game.
Not sure about the impact later on the game :/

for another neat run in a window trick try this:
Download Fallout 1.1 update for dos..
download DOS4GW.exe
download HMIDET.386
download HMIDRV.386
put all this stuff in your fallout directory or a mirror if you prefer..
set up fallout.exe in Dosbox, and run it in a true window with all movies and such intact just like god intended.
All of these files are downloadable at file planet/shack and such..
This does require some knowledge of dosbox, I suggest D-fend Reloaded to make things easier..
I can verify this works under windoze xp..
I have yet to come to terms with vista I'm afraid..

I've actually gotten a number of dos versions of games this way.
The original patches usually contain the full executable, you generally
only need to know how to pass the resource pointers to the exe when
an install routine is unavailable..fallout is a little tricky as it requires the device drivers as well.
warning: I offer no file pointers so use common sense when downloading them. Also, if you find these instructions completely baffleing, you probably should'nt try it anyway ;)
FYI: You may need to set the parameter 'art_cache_size' to 5

The GOG version of Fallout, from what I can tell, is NOT the DOS version. I hope someone comes along and gives us a way to play the DOS version in our DOS application of choice, whether it be Boxer or DosBox and any number of front ends.

The only downside of this is you cannot use any of the community patches. To the OP, what kind of speeds are you getting under Dosbox?

I'm pretty sure there was no DOS version of Fallout that was ever released. And even if there was I don't know why you'd want to play it anyway since the Windows version is just fine.

The DOS version of Fallout comes on the same CD as the Windows version of Fallout. It's an old thread, I know, sorry in advance.
I prefer the DOS version, for two reasons.
#1 - I've been running Linux only for about 4 years now. Even though Fallout2 is Win only and works flawlessly with Wine, using DOSBox is simpler, especially when mounting images is required.
#2 - As much as strange it may seem first, the DOS version (of whatever game) is actually more up to date, as far as ability to run it on new hardware/OS is concerned.
Windows (d)evolves, *nix too, so does Wine and from time to time, compatibility breaks. But DOSBox is still actively maintained, designed to just run these old beauties. The chance of severe compatibility issues between DOSBox and the game is far less likely than with native Windows or Wine. But maybe it's just me having that POV. :)

Fallout 1 Time Limit Patch
I found a way to use them : you rename or delete the file 'VAULT13.GAM' in the folder 'DATADATA'Not sure about the impact later on the game :/

Be very very careful when you use this - I've encountered a lot of bugs and now a game stopping one. I got the chip and yet I can't get inside the Vault - the game acts as if I've already given the chip. This is not the first bug - I've encountered quite a lot of them, and always with something the patch 'fiddled' with - forgotten flags, not taking into account that you may not do things in the order the game expects, etc.
